| Welcome to T. J. Antich, M.S., R.P.T., Inc.
There are several things you should be aware of to insure that your
appointments will run smoothly and on time, to allow the optimum care to be
delivered to you.
|
| 1. MISSED
APPOINTMENTS: If you cannot come in for a scheduled appointment, please call
us at least 24 hours in advance, so that we may schedule someone else needing
our services. Your therapy may be
discontinued if three consecutive appointments are missed without calling, or
you cancel or do not show for half of your appointments within 10 treatment
sessions.

2. ON
TIME: Always report to the receptionist on time for your appointment.
If you come in early, let her know that you are here, as we may be able
to begin a bit ahead of time. If
you are late, we may not be able to see you on that day or may have to render
only a brief treatment so that we may remain on time for the next scheduled
patient. We do our best to run on
time and avoid our patients having to wait for us; likewise, we expect the same
from our patients.

| 3. FRIENDS,
VISITORS AND SPOUSES must wait
in the reception area for patients during their treatments.
Young children must be able to sit quietly in the waiting room, otherwise
we recommend you make other arrangements for their care when you come to
therapy. Our office insurance
policy does not cover non-patients in the treatment areas.

| 4. APPROPRIATE CLOTHING should be worn to allow the physical therapist to adequately examine and treat the area of the body for which you are being seen. | |
| ALL PATIENTS: Low Back, Hip, Knee, and Ankle
Injuries: Patients should wear shorts for exercise unless otherwise allowed by
treating therapist. |
|
| FEMALES: Shoulder, Neck, and Back
Injuries: Patients should bring a sleeveless shirt or tank top.
|
|
| 5. LOCKERS are available in both the men's and women's restrooms.
Keys are available from the front desk receptionist.
Gym patients must store all personal belongings (hats, sweatshirts,
pocketbooks, valuables) in secured lockers.
Clothing and personal items should not be stored in the gym or under
treatment tables as they are a safety hazard.

6. APPOINTMENTS
should be scheduled one week in advance.
This allows our current patients the opportunity to maintain consistent
routines.

| 7. As a sign of courtesy and respect, we will
address our patients as MR.
or MS.
If you desire otherwise, please feel free to inform your therapist.

8. OFFICE
PHONES are for the sole purpose of conducting company business.
Should you need to place a phone call, please use the pay phone in the
reception area.
|
| 9. CELLULAR
PHONES are not permitted to be
on in the facility. Please either
leave them in your car, lock them in a locker, or check them with the
receptionist on your way in. There
has been some discussion about the possible interference of this technology with
medical equipment and we do not wish to compromise the quality or safety of our
treatment to any of our patients.

| 10. There are many
PATIENT RESPONSIBILITIES
that you should be aware of: |
|
| - Please
inform us of your next doctor's visit so that we can have a progress report prepared in advance. |
|
| - Please accurately report changes in your condition from treatment
to treatment. |
|
| - Please give
us 24 hours notice if you are unable to keep a scheduled appointment. Often, there are others on a waiting list who may be able to be seen. |
|
| - Please participate actively in your rehabilitation by performing the
home exercises instructed by your therapist. We can only perform 20 % of the entire result, so you actually play a greater part in success or failure of your treatment. |
|
| - Please notify your therapist of any significant changes in your daily activities such as return to work, beginning or discontinuing any medications.

11. ORTHOPAEDIC
SOFT GOODS recommended by your doctor or therapist may be available for
purchase at our front desk. ALL
PATIENTS are required to pay for the item up front due to the wide variation in
reimbursements by different insurance carriers. The item will appear on your bill and be submitted to your
insurance company with your regular
therapy charges and a reimbursement will be made to you in the event that full
or partial payment is received by us.

| 12. Please remember that your HEALTH CARE INSURANCE
contract is between you and
your insurance company . You
are the one who is receiving service from TJA Inc. and are therefore responsible
for payment. TJA Inc. does not
waive any co-pay or deductible amounts as this constitutes insurance fraud.
Treatment will be discontinued should payment for services (either from
you or your insurance company) not be made in a timely fashion.

| PERSONA |
|
| PRE-CERTIFICATION
OF PHYSICAL THERAPY SERVICES - Following the evaluation, your therapist faxes a
summary of your findings to Personal Choice with a request of the number of
visits deemed to be necessary. It
is our general office practice not to schedule future appointments (after
the initial evaluation) until we have received an authorization from Personal
Choice’s Patient Care Management department.
This avoids placing you in jeopardy of continuing treatments that may not
be covered by your insurance. |

REQUESTS FOR
ADDITIONAL THERAPY - Will be faxed to Patient Care Management on your next to last visit with us, hoping for a response within 48 hours. We encourage you to call them at 1-800-322-2566 should we not receive approval within this time period so as not to cause an unnecessary delay in your treatment. |
|
|
CO-PAYMENT -
Most Personal Choice plans have a $ 10.00 - $ 20.00 co-pay which is due at each treatment session unless alternative arrangements have been made with our office manager, Tom Kincade. We accept cash, check, MasterCard/Visa and American Express for your convenience. |
|
| We are doing
our best to keep up with the latest trends in health care, but it is absolutely
impossible for us to know every detail about every company or plan.
Please remember that Personal Choice is the health care plan that you (or
your employer) selected and that ultimate responsibility for non-covered
services is borne by the patient. In
any case, if you have any questions about your benefits and your
responsibility, JUST ASK ! |

| MEDICARE | |
| Physical
therapy services are covered by “traditional” Medicare as long as treatment
is deemed medically necessary by your referring physician. Medicare patients may be subject to a yearly deductible
either from Medicare or from their secondary insurance. |
|
|
PHYSICIAN
CERTIFICATION OF SERVICES - The 30-day certification period begins the day you see your doctor who prescribes physical therapy treatment for you. Even if you do not begin treatment for several days or weeks, Medicare begins counting the date you saw the doctor or when the prescription is actually dated. |
|
|
ADDITIONAL
PHYSICAL THERAPY TREATMENT - Should you require extended treatment, you must PHYSICALLY RETURN to your doctor for review. Unfortunately verbal or fax re-certifications are not accepted by Medicare. |
|
| WORKERS’ COMPENSATION AND MOTOR VEHICLE ACCIDENT | |
| It is our
office policy to obtain insurance information on prospective patients who are
injured at work or in motor vehicle accidents and confirm benefits with your
adjuster prior to beginning treatment.
This helps us confirm that your
claim is open and allows our administrative staff to properly advise you on your
remaining benefit.
Should this coverage become exhausted, you have the option of continuing
with us according to the terms of your personal health care policy or as a
cash-paying patient should you not have personal health insurance coverage. |

|
| The PERSONALIZED EXERCISE PROGRAM is designed for individuals who wish to engage in a formal exercise program. | |
| An initial
discussion takes place to outline the goals you have set for yourself.
A review of your present medical history is performed by the program
coordinator who then contacts your family physician or internist if any areas of
uncertainty exist. The cost of the
program is paid in advance by the client, and exercise appointments must be
scheduled in order to avoid overcrowding of equipment.
Exercise sessions are scheduled two to three times weekly, and a full
hour should be devoted so that all aspects of the program can be completed. |
